Station 1 — 5 Whys Online Exercise

Discover the Root Cause | Don’t Stop at the Symptom

An interactive root-cause analysis challenge where participants repeatedly ask “Why?” to move beyond the obvious problem and uncover its underlying cause.

Station 2 – Fishbone Wooden Model (Physical Ishikawa Build)

Physical Ishikawa Build | Build the Cause, Find the Root

A hands-on team activity where participants physically build a Fishbone (Ishikawa) model to brainstorm, organise, and analyse the possible causes contributing to a healthcare problem.

Think → Build → Analyse → Identify → Solve

What participants will do:

  • 🦴 Build the physical Fishbone model
